Our complication management protocol
Complications are rare — but how they are managed shows the real level of a clinic. Below is the protocol we apply for filler, botox, mesotherapy, and thread procedures.
Present at every procedure
- Hyaluronidase (Hylenex / Hyalase) — refrigerated, expiry-tracked, minimum 2 vials in stock.
- Chewable aspirin — for suspected vascular events.
- Topical nitroglycerin — for early vasodilation in vascular compromise.
- Warm compress kit, massage gel, oxygen — tissue oxygenation support.
- Epinephrine auto-injector + antihistamine — for anaphylaxis risk.
- Nearest tertiary hospital + emergency department fast-track channel.
Vascular occlusion — the most critical scenario in filler
When hyaluronic acid filler enters a vessel lumen, tissue ischemia, vision loss, or necrosis can develop within minutes. Direct recognition + intervention within the first 90 minutes determines 95% of the outcome.
Early recognition signs
- Unexpected severe pain (beyond normal injection pain)
- Skin blanching or bluish-purple discoloration
- Prolonged capillary refill time
- Vision change — especially after injection in glabella, nose, or forehead
Hyaluronidase protocol
- "High-dose, frequent dosing" principle: 150–300 IU per affected vessel territory, repeat every 60 minutes if needed.
- Direct to the affected area + proximal to the arterial trajectory.
- Concurrently: 325 mg chewable aspirin (if not contraindicated), warm compress, gentle massage.
- If no meaningful improvement at 90 minutes: refer to tertiary center.
Suspected vision loss: Hyaluronidase injection + emergency ophthalmology consult, retrobulbar injection decision by specialist. First 90 minutes are critical.
Botox complications

Patient communication pathway

- Same day: Anything beyond mild swelling. Clinic-hour callback.
- Within 48h: Outcome questions, asymmetry concerns — appointment scheduled.
- Follow-up appointment: Botox week 2; filler weeks 2 and 4; thread weeks 1 and 4.
Follow-up & revision policy
- Botox 2-week touch-up — no charge if needed (asymmetry / insufficient response).
- Filler shape or position revision in weeks 2–4 — massage or hyaluronidase.
- Complication management visits are free of charge; for procedures done at other clinics a referral policy applies.
