Facial Mesotherapy vs PRP: Decision Matrix (2026)
They are not the same thing
In patient language the phrase "skin booster injection" covers both; their preparation, mechanism, and target differ.
| Dimension | Mesotherapy | PRP |
|---|---|---|
| Active substance | Externally prepared cocktail (HA, B-vitamins, peptides, antioxidants) | Platelet-rich plasma separated from the patient’s own blood via centrifuge |
| Mechanism | Substrate feeding — keratinocyte + fibroblast hydration, antioxidant protection | Signaling pathway activation — PDGF, TGF-β, VEGF growth factors driving fibroblast renewal and neocollagenesis |
| Peak result | 2–3 weeks | 4–6 weeks |
| Sessions | 4–6 sessions, 7–14 days apart | 3 sessions, 4 weeks apart |
| Downtime | None — pinpoint redness for 24h | 24–48h mild bruising |
Which one for which complaint
Mesotherapy wins: dull skin; mild dehydration lines (under-eye + cheek); post-smoking or UV damage recovery; spring/autumn maintenance.
PRP wins: structural decline in skin quality; acne scar texture; hair loss adjuvant therapy (separate article); the 35+ patient seeking collagen induction.
Both work: general "skin rejuvenation"; combined with botox + filler — those shape, mes/PRP supports base quality.
Combination protocol — our clinical practice
A three-phase structure we use often:
- Phase 1 (weeks 0–6): Mesotherapy ×3, 2 weeks apart — restore skin hydration and barrier.
- Phase 2 (weeks 8 + 12): PRP ×2, 4 weeks apart — drive new-collagen signaling.
- Phase 3 (months 6 + 12): single maintenance session, depending on skin status.
The logic: passive feeding → active signaling → maintenance.
Quality marker in PRP
Not all "PRP" is equal. Three things determine the result: a closed-kit 2-stage centrifuge (e.g. RegenLab, Endoret); platelet concentration 3–5× baseline; calcium-chloride activation. Patients have the right to ask which kit was used.
Mesotherapy cocktail selection
No single standard. Three skin contexts → three cocktail families: hydration (HA + B5 + glutathione); pigmentation (vitamin C + tranexamic acid + glutathione); structural decline (multivitamin + peptide + acetyl-hexapeptide).
Decision matrix
| Your priority | First choice | Combine? |
|---|---|---|
| Glow + hydration | Mesotherapy | Yes, then PRP |
| Under-eye + fine lines | PRP | Mesotherapy after |
| Acne scar texture | PRP | Add microneedling |
| Seasonal maintenance | Mesotherapy | Alone is enough |
| Botox + filler combo | Both | Sequenced |
| Single choice, tight budget | Mesotherapy | — |
Bottom line
Mesotherapy and PRP are complementary, not substitutes. Which is right for you is decided after a face exam — not by a quick verdict.
